Health 2024 Community Health assessment survey by Cara Jansen Public Health division director welcome thank you for being here thanks for having me um good evening chairwoman and Commissioners um thanks you for allowing me to speak about the these Public Health initiatives that we're working on for 2024 2025 and um any support you can offer is greatly appreciated um for those that may not know I'm Cara Jansen I'm the division director for public health um I'll just go over a quick overview of the process and plan for the county community health assessment and Improvement plan interesting um so our community health goals are to complete uh these two steps the community health assessment and the community health Improvement plan which are required for local Health departments to complete every four years we'll be gathering data with the community health assessment and we'll be putting together the plan with the community health Improvement plan to address um any factors that we identify that contribute to health risks and poor health outcomes com so with the community health assessment we'll be conducting resident surveys we'll be gathering uh data as well as having through focus groups and key informant interviews and then we'll take that data to create the community health Improvement plan which will be our strategic framework for addressing the identified issues for residents in our community the reason that we um are required to do this and why it's very important for public health to focus on the residents is to make sure our heal Improvement efforts are aligned with Community um values and needs we've already launched the survey and we've started to collect the data we've been using our Health Partners through our Public Health Partnership um to help us with this process they helped us um choose our key informants and what groups we'd like to focus on for focus groups as well as review the survey we currently have 50 representatives from a wide variety of community providers that intersect with public health so that includes the schools the colleges um social service providers um homeless Outreach you name it if you'd like a list I'd be happy to share it with you so the reason we need a community health assessment is to make sure we can Target our interventions um to to address the most pressing issues for the members of our community obviously there's a lot of data out there about United States overall or the state of New Jersey but we are really trying to focus on what do our residents need we want to make sure we're focusing on the pressing health concerns for them whether they're chronic health conditions Access to Health Care um Behavior change or social determinance of Health we also make sure that we are allocating our resources where they're needed making sure we make informed decisions for where we spend our funding our time and where we allocate our person now and we also want to make sure that we can measure um the initiatives that we're trying to input into the community so we know where we are with our Baseline we know what Partners do we need to pull in and we know how to measure this over time and the community health Improvement plan will be our next step so after we've gathered the surveys which we'll probably do through the summer and maybe into the fall we can then put together the plan the plan um will be our strategic strategies for addressing any issues um our community members find to be of concern to them we'll develop a road map and we'll develop um how to effectively ensure we can overtime track any improvements also making sure this is stage where we really map out how we're going to use our funding and our staff and we um we'll pull any more Partners in that we may not have already identified so the last time we had a community health assessment or a community health Improvement plan was prior to covid we were in the process of collecting data in 2019 and we had a final report at the beginning of 2020 but we didn't actually create any strategic plans due to um our new focus of addressing uh the covid-19 pandemic so when we look back at our past um goals for the sorry back um our past goals for the uh Community Health Improvement plan residents were looking for um availability of health care services and preventative care and also how to change their Lifestyles to improve their health but really what has happened in the past six years is many different things so we do anticipate there'll be some changes to this so as we all know our Health Care Systems in the area have expanded that's a plus but also you know as long as people can get into to get appointments um that would continue moving that in the right direction covid-19 impacted social determinance of Health for all of our residents whether it was their income maybe their Health Care status changed supports in their Community where they may have had to relocate to um advances in technology um for online information though you can Google anything and get anything posted by anyone um you may not be making the real changes to change your behavior and reduce your risks and Affordable Health Care coverage um there have been many changes over the past six years that have impacted how many people do and do not have insurance so where are we right now in this process we're gathering information um through the survey if you live in Atlantic County you're eligible to participate we've got postcards at the door here as well as on the website we have them all around the county at our partners um there'll be posters up in the libraries we're asking even some businesses where people gather such as a laundromat if they'd be willing to put a poster up for us we have a QR code it's also on the sign out front um so this will this is my push I guess to say anybody and everybody that you may access and have access to we want to get as many voices as possible we want to make sure that we know who's in our community and what their needs are so we want to make sure that every voice is heard and that's why we need everyone um to be participating it's critical for public health to understand the ways in which the social der determinants of Health increase or decrease the risks related to poor health outcomes by measuring Health disparities and inequities will be better prepared to meet people where they are and provide them with resources they'll actually use so we want to develop relevant Services through promoting Equity we want to make sure the people who are living and working in Atlantic County and the people that are actually growing our communities have access to what they need to improve their health outcomes we want to address complex health health issues um we know that the pathways to to individual and Community Health are interconnected with your social structure and your social economic status and we want to create Innovative and inclusive Solutions diverse experiences will provide us the data for understanding population health and developing solutions that support real change currently the survey is going to be AA is available in five languages English Spanish Vietnamese Bengali and Chinese simplified we currently have the English and Spanish postcards we're using grant funding for um translating it into the other three languages and we have a very um helpful intern this year from Stockton who has made some other suggestions for um translations so we'll work with our vendor on um getting the survey translated into other languages and then make the handouts as well so you know I think the best part of this survey is you can start it you can skip responses it is completely Anonymous there's no personal data collected um so I I encourage everyone to just at least give it a try and see what information we are trying to gather does anybody have any questions no I I think uh Cara I mean this this is great because I you know some of the meetings I attend especially through uh you know the mental health Advisory Board these numbers are so important especially for funding they don't have a lot of this data you know about realizing just filling out the forms getting the data getting the numbers really can affect our funding as well for these most important you know health issues that they have so like to sh mam chair yes um on um Atlantic counties U our country is turning 250 years of age in uh 2026 and uh many of us went to the seminars that they had at the Jersey Association of counties and of course one of them was put on by the county of mm and they're doing quite an extensive uh celebration um I don't anticipate that we could even approach their their intensity uh their budget and uh they have a a full-time person uh preparing for this event many which includes many many many events along the year 2026 but I would hope that we could do something here at Atlanta County uh celebrating our 250 years as a um the only battle that we had in Atlanta County was the Battle of chest on neck most most of you know where that is and what Republic area and if you can't sleep some night you can watch me giving a talk on the Battle of CH neck on YouTube but U it's important that you know we as a county celebrate take part in this celebration as well um and I would hope that um maybe Kim Brown could possibly head head something up and get in contact with the um s sons of American Revolution d uh other uh veterans uh groups to see where we can go with this and what we can do here in Atlanta County make it special for us as well thank you thank you follow up on that Gallow Township is 2 years older than the country so this this year is the 250th anniversary of the township and was at a meeting last night and they're talked about all kinds of events in Galloway adding to the incredible Fourth of July parade but they're dealing events all summer while to commemorate Gallow times of 250th anniversary good potentially including fireworks on the fourth first don't hold me to that but okay um but they have a lot of events um planned for that celebration as well oh nice keep this updated mam chair one more thing I guess I'll say as an education School side I want to give a special congratulations to all the new graduates of ACCC a few of the Commissioners were there this young man next to me had a double reason to be there so congratulations to you and your family as well and Dr Gala and everyone at ACCC and let us not uh forget our commissioner G being
